Drug and alcohol policy builder

If you have a CDL driver — including yourself — federal law says you must hand them a written policy covering twelve specific things, and keep their signed receipt. There is no government form for it. This builds both.

01FMCSA does not publish a template. The closest thing is a guidance chapter last updated in March 2014 — which predates the Clearinghouse and is missing a required item entirely.
02Every section is labelled with the paragraph of 49 CFR 382.601(b) it satisfies, so you and an auditor can both check the work.
03Nothing you type leaves your device. No account, no database, no email required.
04You get two documents: the policy, and the signed receipt form — one per driver. The receipt is the part carriers forget and the part auditors ask for.

Tell us about your company

Six required fields. Everything else has a sensible default.

The carrier

Exactly as it appears on your FMCSA registration.

Who runs the program

Must be an employee of the company. A consortium or service agent legally cannot fill this role.

If you are a single-driver operation you are required to be in a consortium — you cannot run a random pool of one.

Two optional additions

A driver who comes forward before reporting for duty can get evaluation and treatment without it becoming a DOT violation — but only if you have a written program. 49 CFR 382.121

Anything here gets printed in its own clearly marked section. 49 CFR 382.601(c) requires that policies based on your own authority be clearly and obviously separated from what DOT requires — mixing them is a violation on its own.

Your policy & receipt

Before you hand this out — three things

Read it. It is your policy now, and you are the one an auditor will ask about it. If anything in it does not describe how you actually operate, change it.

Print two things. The policy goes to every driver. The receipt on the last page gets signed by each driver and you keep it — not them. Print one receipt per driver.

Keep the signed receipt for as long as that driver works for you, plus two years. 49 CFR 382.401(b)(4), (c)(5)(ii)

What this does not do

This builds the policy document. It does not enroll you in a consortium, run your random selections, train your supervisors, or register you in the Clearinghouse. Those are four separate things, and having a beautiful policy without them is how carriers fail a new entrant audit while feeling prepared.
